Confidential, DOT-compliant evaluations and return-to-duty services in Fort Worth and via secure telehealth.
When a safety-sensitive employee violates U.S. Department of Transportation drug and alcohol regulations, federal law requires evaluation by a qualified Substance Abuse Professional before that employee can return to duty. Guided Purpose Counseling provides these evaluations with the clinical rigor the process demands and the respect every person deserves.
As a qualified Substance Abuse Professional (SAP), Shanelle Peoples-Lambert is authorized to assess, evaluate, and recommend treatment and education for individuals who have violated DOT regulations under 49 CFR Part 40 — and to make the final determination of compliance before return to duty.
What the SAP process involves
The DOT return-to-duty process follows a federally defined sequence, and we guide you through every step: a comprehensive initial face-to-face evaluation to assess the situation and determine what’s required; a referral to appropriate education or treatment tailored to your circumstances; after you complete those recommendations, a follow-up evaluation to confirm you’ve followed through; and finally, return-to-duty documentation and a follow-up testing plan provided to your designated employer representative, as required by DOT.
Who we serve
We work with individuals referred through DOT-regulated industries — including trucking and commercial driving (FMCSA), aviation (FAA), railroads (FRA), transit (FTA), pipeline (PHMSA), and maritime (USCG) — as well as the employers, Designated Employer Representatives (DERs), Employee Assistance Programs (EAPs), and treatment facilities who refer them.
For employers, EAPs, and referral partners
If your organization needs a reliable SAP for DOT-regulated employees, we offer prompt scheduling, clear communication, and documentation that meets federal requirements. We’re available to establish ongoing referral relationships and contracted EAP arrangements. Reach out to discuss how we can support your team.
Fees
This service is private pay or covered under contracted Employee Assistance Programs (it is not billable to standard health insurance, per DOT rules).
Initial SAP Evaluation: $300 — DOT-compliant substance use evaluation with an individualized treatment/referral plan.
Follow-Up Evaluation: $300 — treatment/referral review, follow-up evaluation, and return-to-duty documentation.

Evaluations are available in person at our Fort Worth office and via secure, HIPAA-compliant telehealth for clients located in Texas, consistent with current DOT policy on remote SAP evaluations.

Board-approved clinical supervision for LPC Associates in Texas — in person in Fort Worth and via secure telehealth.
Becoming a fully licensed professional counselor takes more than a degree. Texas requires LPC Associates to complete supervised clinical experience under an approved supervisor before independent licensure. The right supervisor doesn’t just sign off on hours — they help you become the clinician you want to be.
At Guided Purpose Counseling, supervision is offered by Shanelle Peoples-Lambert, LPC-S, an LPC Supervisor approved by the Texas Behavioral Health Executive Council, with 18 years in the helping profession and specialized experience in substance use, trauma, and addiction counseling.
My supervision philosophy
The themes that guide my clinical work — Meaningful Progress and Progress is not Perfection — guide my supervision, too. My goal isn’t to make you a copy of me; it’s to help you find your own clinical voice while meeting the standards set by the Texas State Board. Supervision should be a space where you can bring your hardest cases, your uncertainties, and your growth edges without fear of judgment.
What you can expect
Regular individual supervision sessions focused on your clinical development, case consultation, and the competencies Texas requires. We’ll work on clinical skills and case conceptualization, ethical and legal practice, documentation, professional identity and development, and — if it aligns with your interests — specialized areas such as substance use and addiction counseling, where I hold a Master Addiction Counselor credential and serve as a DOT Substance Abuse Professional.
Who this is for
LPC Associates in Texas who are accruing supervised hours toward full licensure and want a supervisor invested in their growth. Whether you’re just beginning your associate period or further along and looking for a strong fit, reach out to see if we’d work well together.
Fee
Individual Clinical Supervision: $240 per month.

A nonverbal, calming complement to therapy and recovery — offered by a certified Acudetox Specialist in Fort Worth.
Healing isn’t only a conversation. AcuDetox is a gentle form of auricular (ear) acupuncture that supports your nervous system alongside the deeper work you do in therapy. At Guided Purpose Counseling, it’s offered as a complement to counseling and substance use treatment — another pathway toward meaningful progress. Whether you’re working through recovery, carrying chronic stress, or simply looking for a moment of calm, AcuDetox meets your body where talk therapy meets your mind.
What AcuDetox is
AcuDetox uses the NADA protocol, a standardized treatment developed by the National Acupuncture Detoxification Association and used in behavioral health, addiction recovery, trauma response, and stress-relief settings for decades. It involves the gentle placement of up to five small, sterile, single-use needles at specific points on each outer ear. The five points — known as Sympathetic, Shen Men, Kidney, Liver, and Lung — are associated in this tradition with relaxation, calm, and overall balance. Treatment is provided by Shanelle Peoples-Lambert, a certified Acudetox Specialist (ADS) with 18 years in behavioral health and deep experience in substance use recovery.
What it’s used to support
AcuDetox is a supportive, complementary therapy. People incorporate it into their care to help with:
Relaxation and everyday stress relief
Cravings and withdrawal discomfort during substance use recovery
Sleep difficulties and restlessness
Anxiety, tension, and emotional overwhelm
A general sense of calm, focus, and centeredness
Because it requires nothing to be said and no homework to complete, many people find it a welcome counterpart to talk therapy — a chance to simply rest and reset. It works best alongside counseling, medical care, and a recovery plan, not as a replacement for them.
What a session is like
You’ll sit comfortably in a quiet, dimly lit space — individually or, when offered, in a small calm group. After the needles are placed, you simply relax for about 30 to 45 minutes; many people rest, breathe, or even doze. There is nothing you need to do or talk about. When the session ends, the needles are removed and safely discarded. Most people leave feeling noticeably more settled. Effects are often cumulative, so a series of sessions tends to be more supportive than a single visit.
What to expect at your first visit
Before your first AcuDetox session, we’ll briefly review your goals and relevant health history to make sure it’s a good and safe fit for you. We’ll explain the process, answer your questions, and only begin when you feel comfortable. If you’re already a counseling client, AcuDetox can be woven into your existing care; if you’re coming specifically for AcuDetox, we’ll talk about how it fits your broader wellness or recovery plan.
Who it’s for
Clients in substance use recovery seeking added support, anyone managing stress, anxiety, or sleep difficulties, and people who want a drug-free, body-based complement to talk therapy. Because AcuDetox involves needles and gentle physical treatment, we’ll review your health history first — certain conditions may call for clearance from your physician, and we’ll guide you on that before beginning.

Reconnect, communicate, and grow — together. In-person in Fort Worth and via secure telehealth.
Every relationship moves through seasons — some easy, some hard. When communication breaks down, trust is shaken, or you simply feel more like roommates than partners, counseling offers a space to understand each other again and decide, together, where you want to go. At Guided Purpose Counseling, we help partners do the honest, hopeful work of reconnection, guided by the belief that progress — not perfection — is what moves a relationship forward.
What we work on
Couples come to us at all stages — dating, engaged, married, or rebuilding. Common focuses include:
Communication patterns and recurring conflict
Rebuilding trust after betrayal or breaches of confidence
Emotional disconnection and feeling unseen or unheard
Major life transitions — parenthood, relocation, career change, empty nest
Parenting and blended-family stress
Differing needs around closeness, independence, and intimacy
Premarital preparation for couples building a foundation
Our approach
We provide a balanced, non-judgmental space where both partners feel heard — not a referee’s chair where someone is declared right or wrong. Together we’ll identify the patterns underneath the arguments, build practical communication and repair skills, and reconnect you to what brought you together. Treatment is collaborative and tailored to your relationship’s goals.
When intimacy or sex is part of the picture
Relationship struggles and sexual concerns often overlap. If desire differences, intimacy, or sexual difficulties are part of what you’re navigating, we can address them within couples work — or, when more focused support is helpful, through our dedicated Sex Therapy services. (See our Sex Therapy page.)
Who it’s for
Partners of any age, orientation, or relationship structure who want to communicate better, heal, and reconnect. Individuals working on relationship patterns are welcome too — you don’t always need both partners in the room to begin.

Confidential, judgment-free support for your sexual health and intimacy — for individuals and couples, in person in Fort Worth and via secure telehealth.
Sexual concerns are common, valid, and treatable — yet they’re among the hardest things to talk about. Sex therapy offers a safe, professional space to explore desire, intimacy, and sexual well-being without shame or judgment. At Guided Purpose Counseling, this work is offered with the specialized training of a Certified Sex Therapist Candidate, completed under the clinical supervision of Dr. Ashley Staats, LMFT-S, CST.
What sex therapy is — and isn’t
Sex therapy is talk therapy. It involves conversation, education, and practical strategies — there is no physical examination, nudity, or physical contact of any kind. It’s a confidential, collaborative process focused on understanding your concerns and helping you move toward the intimacy and well-being you want.
What we help with
People seek sex therapy for many reasons, including:
Low or absent desire, and desire differences between partners
Arousal and orgasm difficulties
Pain during sex
Out-of-control or compulsive sexual behavior
Rebuilding intimacy after trauma, illness, surgery, or medical change
Mismatched expectations, performance worries, and rebuilding confidence
Exploring sexual identity, orientation, and self-understanding
Reconnecting sexually in a long-term relationship
Individual or with a partner
Sex therapy works whether you come on your own or with a partner. Many concerns are deeply personal, and you don’t need to be in a relationship — or bring your partner — to get meaningful help.
Our approach
We combine evidence-based sex therapy with a warm, affirming, sex-positive stance that welcomes clients of all identities, orientations, and relationship structures. Together we’ll understand what’s happening, address the physical, emotional, and relational factors involved, and build a path forward grounded in meaningful progress — not perfection.
